An AI answer is produced in three stages, and almost every visibility problem can be located in one of them. Retrieve is where the bot crawls and pulls content. Augment is where trust signals decide which passages are worth quoting. Generate is where the answer is written and the chosen sources are cited. Diagnosis starts by asking which stage failed, because the fix is completely different in each.
